The Kingdom of Solmara is at war with the Iron Concord, and the border garrisons are falling silent—not to death, but to an unnatural sleep. You are agents of the Crown, sent to investigate the watchtower at Hearthglen, where no messenger has returned for three days. What you find is a garrison of sleeping soldiers, a sweet-smelling mist, and a conspiracy that reaches into the royal court itself. The Numb Crown is a grim mystery of royal intrigue, alchemical anesthesia, and a war that someone wants to freeze in place.

Act I begins with a single silent tower and ends with the first hint of a world-clasping plot: a shadowy patron called the Pale Regent, a Quiet Choir, and a formula that could put entire nations to sleep.

Player Handout

War has raged between Solmara and the Iron Concord for two years. The border is a meat grinder of mud, arrows, and exhausted levies. But three days ago, the watchtower at Hearthglen stopped sending riders. No smoke, no signal, no bodies. The Crown's spymaster has no soldiers to spare, so she is sending you—a small, deniable team—to find out what happened. You arrive at dusk in cold rain. The gate is unbarred.

The guards are asleep at their posts, breathing slowly, faces peaceful. A sweet, cloying smell hangs in the air. Somewhere below the tower, a machine hums. You do not know yet that this is the first breath of a plot to put the world to sleep. You only know that something is very wrong, and that the war outside is not the only thing that can kill you.

The Numb Crown: Act I — The Sleeping Garrison

Behind the screen

This free preview contains Act I of The Numb Crown, a 5e campaign about a royal conspiracy to end war by anesthetizing the world. The party begins at 1st level and reaches 3rd level by the act's end. Act I is set at Hearthglen Watchtower and its underground barracks, where the Royal Anesthetist Ysolde Marr is testing a gas called Quiet Breath on captured soldiers. Ysolde is the act's central villain, but she is not the ultimate enemy—she is a talented, desperate pawn of a larger power.

The true big bad, the Pale Regent, remains off-screen, hinted at through sealed orders, a closed-eye crown sigil, and references to the Sleepless Court. The act's secrets: the war is being prolonged by the Quiet Choir, a network of royal physicians and alchemists who believe numbness is mercy; the Crown may already be compromised; and the formula for Quiet Breath is only one piece of a larger anesthetic weapon. Progression is milestone-based.

The party should investigate the watchtower, survive the barracks guardians, confront Ysolde in her surgical theater, and recover the sealed orders that point toward the capital. The tone is tense, eerie, and politically charged, with body-horror-adjacent imagery but no graphic gore. The act is designed for 3–5 characters and 3–4 sessions of play.

Adventure Hooks

Hook 01

The Crown's Deniable Agents

Spymaster Maera Vell summons the party to a rain-soaked war room. The border watchtower at Hearthglen has gone silent for three days, and she has no soldiers to spare. She offers coin, royal favor, and a chance to serve Solmara in a way that will never appear in official records. The hook is simple: go to Hearthglen, find out what happened, and bring back proof.

Hook 02

The Sleeping Garrison

Refugees and deserters whisper that Hearthglen's guards were not killed but cursed to sleep where they stood. A sweet smell hangs over the tower, and no one who enters returns with a clear story. The party is drawn by duty, curiosity, or the promise of reward—but the truth is an alchemical anesthetic called Quiet Breath, and it is only the first breath of a much larger plot.

Act Structure

Act I: The Sleeping Garrison

Recommended level: 1-3

The party arrives at Hearthglen Watchtower to find the garrison asleep, not dead. They investigate the tower and barracks, discovering evidence of an alchemical anesthetic called Quiet Breath. Below the barracks, they confront the Royal Anesthetist Ysolde Marr, who is experimenting on a captured scout. Ysolde is a mid-tier villain and a pawn of the Pale Regent. The party can defeat or drive her off, save the garrison, and recover sealed orders that point toward the Sleepless Court and a world-clasping conspiracy.

Objectives

  • Investigate the silent watchtower and determine why the garrison has fallen asleep.
  • Navigate the barracks and bypass or defeat the animated guardians.
  • Confront Ysolde Marr in the underground surgical theater and stop her experiments.
  • Recover the sealed orders and learn the names 'Pale Regent' and 'Quiet Choir'.
  • Save the sleeping soldiers and the captured scout if possible.
  • Reach 3rd level and gain a lead toward the capital and Act II.

The Silent Watchtower

Hearthglen Watchtower, Solmaran border

Read aloud

Rain hisses against the watchtower's stone. The royal banner hangs limp and heavy. No challenge comes from the gate. Through the arrow slits, you see a guard slumped over a crossbow, chest rising and falling in unnatural sleep. A sweet, cloying smell drifts from the cellar grate.

Running this scene

The watchtower is quiet but not empty. Six soldiers are asleep at their posts, under the effect of Quiet Breath. They are not dead and will not wake without an antidote or a successful Medicine check. The sweet smell comes from the cellar. There are no enemies in this scene unless the party makes a loud disturbance; if they do, 2 Guard (SRD) stumble out in a sleepwalking daze, non-hostile but confused.

The real danger is the lingering gas: any character who spends more than 10 minutes in the cellar must succeed on a DC 11 Constitution saving throw or gain one level of exhaustion. The scene is investigation-focused. Encourage the players to examine bodies, search the tower, and follow the smell.

Checks
  • Investigation DC 12: Searching the watchtower desk reveals a half-empty vial labeled 'Quiet Breath' and a shipping crate stamped with a royal seal—a crown with a closed eye.
  • Medicine DC 13: Examining a sleeping soldier confirms the condition is alchemical anesthesia, not poison or disease. The soldier can be roused with an antidote or a strong stimulant.
  • Perception DC 10: A faint rhythmic humming comes from beneath the cellar grate, along with the clink of glass and metal.
  • Arcana DC 14: The greenish residue on the vial is a complex anesthetic compound, likely requiring a trained alchemist or royal physician to manufacture.
Clues
  • Empty vial labeled 'Quiet Breath'.
  • Royal seal of a crown with a closed eye on a shipping crate.
  • Boot prints leading from the cellar to the barracks and back.
  • A logbook entry noting 'shipment to Hearthglen—priority, Anesthetist Royal'.
Where next
  • The trail of boot prints and the humming lead down into the barracks cellar. The party can descend through the tower or the exterior cellar doors.

The Dreaming Barracks

Hearthglen Barracks and Cellar

Read aloud

Rows of cots fill the barracks. Soldiers sleep in full armor, faces slack. A faint hum pulses from a locked door at the far end. Two suits of ceremonial armor stand guard, their visors dark—until one turns its head.

Running this scene

The barracks is filled with sleeping soldiers. The locked door at the far end is protected by an alarm ward and two Animated Armor and one Flying Sword (SRD). These guardians were left by Ysolde to deter intruders. They attack anyone who approaches the door without disabling the ward. If the party disables the ward with Arcana DC 14, the guardians remain inert. If combat begins, the Animated Armor block the path while the Flying Sword flanks.

The room is cramped, with cots providing difficult terrain. A successful Stealth check can let the party avoid notice and reach the door, but the ward still triggers if not disabled. The door is locked; it can be forced with Athletics DC 12 or picked with Thieves' Tools DC 13.

Checks
  • Stealth DC 13: Moving quietly through the barracks avoids waking the guardians prematurely.
  • Arcana DC 14: Disabling the alarm ward prevents the guardians from animating.
  • Athletics DC 12: Forcing the locked door open without tools.
  • Investigation DC 12: Searching the barracks office reveals a logbook with troop movements and a map of border forts marked with closed-eye sigils.
Clues
  • Logbook with troop movements and a map of border forts.
  • Note reading 'The Regent's schedule is set. Hearthglen is the first.'
  • A requisition order for large quantities of 'Quiet Breath' sent to other garrisons.
  • A gray hood left behind, smelling of chemicals.
Where next
  • Beyond the locked door, stairs spiral down toward a chemical reek and a soft, rhythmic chanting. The party can descend to the surgical theater.

The Anesthetist's Theater

Underground Surgical Theater beneath Hearthglen

Read aloud

A domed chamber of white tile and brass valves. A captured scout lies strapped to a table, a mask over his face. A woman in a physician's coat turns, vial in hand. 'You are early,' she says. 'The war is not yet asleep.' Behind her, two acolytes in gray hoods prepare more gas.

Running this scene

This is the act's central confrontation. Ysolde Marr, the Royal Anesthetist, is the villain. She is not a monster; use her forthcoming character sheet for her statistics and actions. She is accompanied by 2 Cultist and 1 Thug (SRD). Ysolde fights from the back, using anesthetic gas and alchemical devices. The cultists cast spells and protect her, while the thug blocks the stairs. If reduced to half her hit points, Ysolde tries to escape by opening a valve that fills the room with Quiet Breath.

Any creature in the room must succeed on a DC 13 Constitution saving throw or fall unconscious for 1 minute. A character can shut the valve with a Dexterity check DC 13. Ysolde will surrender if clearly outmatched, offering information in exchange for her life—but she lies about the Pale Regent's identity. The captured scout is alive but unconscious; he can be saved with a Medicine check DC 13 or an antidote.

Checks
  • Persuasion DC 15: Making Ysolde hesitate by appealing to her oath as a physician. On a success, she pauses for one round and orders her acolytes to stand down, allowing a parley.
  • Investigation DC 14: Searching the theater reveals a ledger of shipments to other garrisons and a sealed scroll case on her desk.
  • Dexterity DC 13: Shutting the gas valve before the room fills with Quiet Breath.
  • Medicine DC 13: Stabilizing the captured scout and identifying the gas as a refined version of Quiet Breath.
Clues
  • Letter from 'The Pale Regent' mentioning 'the world will sleep, and the war will end'.
  • Ledger of Quiet Breath shipments to at least three other border garrisons.
  • Royal sigil of a crown with a closed eye on Ysolde's coat.
  • Ysolde's muttered reference to 'the Sleepless Court' and 'the Quiet Choir'.
Where next
  • With the valve shut and Ysolde defeated or fled, the theater falls silent—except for a sealed scroll case on her desk. The party can recover it and return to the surface.

The Sealed Orders

Ysolde's Study, Hearthglen

Read aloud

The scroll case bears the royal seal—a crown with a closed eye. Inside, a single page: 'To the Anesthetist Royal: The Quiet Choir convenes at the Sleepless Court. Bring the final formula. The world will not wake until it is ours.' It is signed with a sigil you do not recognize: a pale hand over a crown.

Running this scene

This is the aftermath and cliffhanger. The party can save the scout, wake the soldiers with an antidote, and search Ysolde's study. There is no combat unless Ysolde escaped earlier and returns with 2 Guard (SRD) for a final desperate attack. The sealed orders are the key clue. They reveal the names 'Pale Regent' and 'Quiet Choir' and the location 'Sleepless Court'. The party should realize Ysolde was a pawn, not the mastermind.

The Pale Regent is the campaign's big bad, but remains off-screen. The act ends with the party reaching 3rd level and gaining a clear lead to the capital. If the party failed to stop Ysolde, she escapes and the orders are still found, but she becomes a recurring villain.

Checks
  • Medicine DC 13: Brewing an antidote from Ysolde's supplies to wake the sleeping soldiers.
  • Investigation DC 12: Decoding the pale hand sigil as a mark of the Pale Regent, a figure not recognized in Solmaran heraldry.
  • Insight DC 14: Realizing Ysolde was terrified of her patron and believed she was saving lives by ending the war through anesthesia.
  • History DC 13: Recalling that the Sleepless Court is a ruined royal retreat in the capital, long sealed after a plague.
Clues
  • Sealed orders naming the Pale Regent and the Quiet Choir.
  • Location: the Sleepless Court in the capital.
  • Reference to a 'final formula' that can put entire armies to sleep.
  • Ysolde's journal expressing doubt and fear of 'the Regent's eyes'.
Where next
  • The trail leads to the capital, where the Queen's court sleeps uneasily—and the war is about to take a darker turn. Act II awaits.

Milestone: Defeating or driving off Ysolde Marr, saving Hearthglen's garrison, and recovering the sealed orders. The party reaches 3rd level and gains a clear lead to the Sleepless Court and the Pale Regent.

Villain Plans

Ysolde Marr Character Sheet

Royal Anesthetist, Pawn of the Pale Regent

Motivation

Ysolde believes the war is a slaughterhouse and that numbness is mercy. She was recruited by the Quiet Choir after she lost her brother to a pointless border skirmish. She now serves the Pale Regent because they promised to end the war by putting entire armies to sleep. She is desperate, guilt-ridden, and terrified of her patron, but she genuinely believes she is saving lives.

Personality

Clinical, soft-spoken, and precise. Ysolde speaks in medical metaphors and rarely raises her voice. She is not cruel for cruelty's sake; she sees her victims as patients who need rest. When cornered, she becomes cold and bargaining, offering information only to buy time or escape.

Resources

  • Quiet Breath gas and refined anesthetic vials
  • Two loyal cultist acolytes
  • A thug bodyguard
  • A sealed scroll case with orders from the Pale Regent
  • A ledger of Quiet Breath shipments to other border garrisons

Battle Plan

Ysolde begins combat from behind her surgical table, using anesthetic gas and alchemical devices to incapacitate the party. She orders her cultists to protect her and the thug to block the stairs. If reduced to half her hit points, she opens the main gas valve to fill the room with Quiet Breath (DC 13 Constitution save or unconscious for 1 minute). She tries to escape through a back passage if the valve is shut or her minions fall. If clearly outmatched, she surrenders and offers false information about the Pale Regent.

Defeat or Redemption

If defeated, Ysolde can be captured and interrogated. She will reveal the names 'Pale Regent' and 'Quiet Choir' and the location of the Sleepless Court, but she lies about the Regent's identity to protect her own life. If the party appeals to her oath as a physician with Persuasion DC 15, she hesitates and may stand down for one round, allowing a parley. She cannot be fully redeemed in Act I, but she can become a reluctant informant. If she escapes, she becomes a recurring villain who may seek revenge or redemption in later acts.

Running Notes

Recommended party: 3–5 characters, 1st level, with a balanced mix of martial, divine, arcane, and skill-focused roles. A character with Medicine, Arcana, or alchemist's supplies will be especially useful. A face character helps in the final confrontation, and a durable front-liner can hold off the barracks guardians.

Session Zero

Before play, discuss the campaign's themes: war, medical horror, anesthesia, and political conspiracy. Confirm that everyone is comfortable with descriptions of unconsciousness, restraint, and alchemical experimentation. Establish that the party are agents of the Crown, so they have a reason to work together and follow orders. Ask each player to choose one reason their character distrusts the Crown or the war. The party should include at least one character with Medicine, Arcana, or alchemist's supplies. Starting level is 1, and the party will reach 3rd level by the end of Act I.

Scaling Guidance

The encounters are designed for 3–5 characters of 1st to 3rd level. For a larger party, add one additional official monster of the same type or increase the villain's hit points by 25%. For a smaller party, remove one minion from the Theater encounter or allow the party to disable the barracks alarm ward with a lower DC 12 Arcana check. If the party is struggling in the Theater, Ysolde's gas valve can malfunction and affect only her own minions. If the party is breezing through, Ysolde can use a second vial of Quiet Breath and the thug can gain advantage on attacks against a grappled target.

DM Advice

Act I is investigation-heavy at the start and combat-heavy at the end. Let the players explore the watchtower and barracks at their own pace. The sweet smell of Quiet Breath should be a constant sensory detail. The sleeping soldiers are not dead; the party can save them with an antidote or a successful Medicine check. Ysolde Marr is the central villain of this act, but she is a pawn. The Pale Regent remains off-screen and should only be hinted at through sealed orders, the closed-eye crown sigil, and references to the Sleepless Court. Do not reveal the Pale Regent's identity in Act I. If the party fails to stop Ysolde, she escapes and the sealed orders are still found, but she becomes a recurring villain. End the act with the party reaching 3rd level and gaining a clear lead to the capital.

Secrets & Clues

  • Empty vial labeled 'Quiet Breath' found in the watchtower desk.
  • Royal seal of a crown with a closed eye on a shipping crate.
  • Boot prints leading from the cellar to the barracks and back.
  • A logbook entry noting 'shipment to Hearthglen—priority, Anesthetist Royal'.

Encounters

Sleepwalking Guards at the Gate

Suggested Level: 1

Two guards stumble out of the watchtower in a sleepwalking daze, confused and non-hostile unless the party makes a loud disturbance or attacks them. They mutter about a sweet smell and a humming below the cellar.

Creatures

Terrain Features

Rain-slick stone gate, narrow arrow slits, a heavy portcullis that is already half-raised, and a cellar grate leaking green mist.

Treasure

None. The guards carry standard spears and shields, but their equipment is royal issue and not worth stealing.

DM Notes

This encounter is optional and should not punish stealth or diplomacy. If the party calms the guards with Persuasion DC 12 or Medicine DC 12, they can learn that the last thing the guards remember is a physician in a gray hood checking the cellar.

The Dreaming Barracks Guardians

Suggested Level: 1-2

The barracks is filled with sleeping soldiers. A locked door at the far end is protected by an alarm ward and guarded by two suits of animated ceremonial armor and a flying sword. These guardians attack anyone who approaches the door without disabling the ward.

Creatures

Terrain Features

Rows of cots create difficult terrain. The alarm ward glows faintly above the locked door. A narrow side office contains a logbook and a gray hood.

Treasure

A logbook with troop movements, a map of border forts marked with closed-eye sigils, and a gray hood smelling of chemicals.

DM Notes

The party can avoid combat by disabling the alarm ward with Arcana DC 14. If combat begins, the Animated Armor block the path while the Flying Sword flanks. The locked door can be forced with Athletics DC 12 or picked with Thieves' Tools DC 13.

The Anesthetist's Theater

Suggested Level: 2-3

DM Notes

This is the act's central confrontation. Ysolde is not a monster; use her forthcoming character sheet. If reduced to half her hit points, she tries to escape by opening the gas valve. Any creature in the room must succeed on a DC 13 Constitution saving throw or fall unconscious for 1 minute. A character can shut the valve with a Dexterity check DC 13.
