The Four-Headed Storm

Welcome to Tidewatch, a bustling temperate coastal city where the salt air mixes with the scent of adventure. You are a party of four adventurers, each with your own reasons for being at the docks on this fateful day. A ship, the *Sea Serpent's Bane*, has just returned after being lost for over a year. But something is terribly wrong: the crew is fighting, leaping overboard, or staring blankly at nothing. Only the captain seems lucid.

You will soon discover that the ship brought back a cursed idol—one of many artifacts being created by a lich named Malakar the Soulbinder. He seeks to summon the Cataclysm, a four-headed dragon representing the four elements, to control the world. Your journey will take you from the sewers beneath Tidewatch to distant biomes, from whimsical wizard towers to elemental shrines. Along the way, you'll meet eccentric characters, tackle silly side quests, and uncover the first hints of a conspiracy that threatens all existence.

This campaign is high fantasy with a dark premise, but it's designed to be light-hearted and full of memorable, often ridiculous, moments. The world is your sandbox—explore it, get sidetracked, and have fun. The storm is coming, but there's always time for a detour.

The Four-Headed Storm

Behind the screen

Act I: The Haunted Tide (Levels 3–5)

This free preview covers the opening act of *The Four-Headed Storm*. The party begins at level 3 in the coastal city of Tidewatch. The adventure kicks off when the *Sea Serpent's Bane* returns after a year lost at sea, its crew driven mad by a Mind-Rending Idol. The party must contain the chaos, investigate the idol, and trace it to a cult operating in the city's sewers.

Key Secrets:

  • The idol is one of many created by Malakar the Soulbinder, a lich who seeks to summon the Cataclysm, a four-headed elemental dragon.
  • The cult, known as the Cult of the Four Winds, is seeding these idols across the land to gather elemental energy.
  • The cult leader in Tidewatch is Kaelen the Ember, a fanatic who wields a fire-themed idol. He is a pawn, not the ultimate villain.
  • A letter from Malakar reveals a contact in the city of Emberfall and mentions "the Nexus"—the site of the summoning.

Progression: The party should reach level 5 by the end of this act. They will have a clear lead to pursue in Act II. The tone is high fantasy with whimsical NPCs and side quests to balance the dark themes. Estimated playtime: 3–4 sessions of 3–4 hours each.

Adventure Hooks

Hook 01

The Returning Ship

You are at the docks of Tidewatch when the Sea Serpent's Bane, lost for over a year, drifts into port. The crew is violently insane or catatonic. Captain Elara Voss, the only lucid soul aboard, calls for help. You are level 3 adventurers—perhaps locals, mercenaries, or scholars—drawn by the commotion.

Hook 02

A Captain's Plea

Captain Elara Voss desperately seeks aid to contain her cursed crew and dispose of a dangerous artifact. She promises reward and answers. This hook works for characters with nautical, mercenary, or scholarly backgrounds.

Act Structure

Act I: The Haunted Tide

Recommended level: 3-5

The party witnesses the return of the Sea Serpent's Bane, a ship lost for over a year. The crew is afflicted by a cursed idol that rends minds. They must contain the madness, uncover the idol's origin, and trace it to a cult operating in the city's underbelly. They confront a cult leader who is using the idol to spread chaos, and learn of a larger conspiracy involving a lich and elemental artifacts.

Objectives

  • Secure the docks and the ship, subduing or calming the afflicted crew.
  • Investigate the cursed idol and learn its nature.
  • Track the idol's origin to the Cult of the Four Winds hideout.
  • Defeat the cult leader Kaelen the Ember and recover information about the lich Malakar.

The Return of the Sea Serpent's Bane

The Docks of Tidewatch

Read aloud

The fog parts to reveal a battered ship, its sails torn. The Sea Serpent's Bane, lost for over a year, drifts into port. Even from the dock, you can hear screams and the clash of steel. Sailors leap overboard into the cold water, while others stand frozen, staring at nothing. Only the captain, a tall woman with a bloodied cutlass, seems aware of her surroundings.

Running this scene

The ship is the Sea Serpent's Bane, captained by Elara Voss. The crew is affected by a cursed idol stored in the captain's cabin. As the ship docks, 3 Commoner (maddened sailors) are fighting each other or trying to jump overboard, and 1 Thug (the first mate, driven violent) attacks anyone who approaches. Captain Voss tries to restore order but is overwhelmed. The party can fight, subdue, or use skills to calm the sailors.

The idol's influence is a Wisdom saving throw (DC 13) for anyone within 30 feet of the cabin, causing confusion as the spell. The captain has locked the idol in a lead-lined chest, which blocks most of its effect. She will ask the party for help once the immediate chaos is contained.

Checks
  • Strength (Athletics) DC 12 to restrain a panicked sailor without hurting them.
  • Charisma (Persuasion) DC 13 to calm a sailor long enough to listen.
  • Wisdom (Insight) DC 14 to realize the sailors are not hostile but magically confused.
  • Dexterity (Acrobatics) DC 12 to avoid being pulled overboard by a leaping sailor.
Clues
  • The sailors mutter about a 'four-faced stone' and a 'song that never ends'.
  • Captain Voss mentions they found the idol on an uncharted island called the Isle of Whispers.
  • The idol is locked in a lead-lined chest in the captain's cabin.
Where next
  • Once the crew is subdued, Captain Voss begs the party to accompany her to her cabin to examine the idol.
  • Alternatively, the party can investigate the ship on their own and find the cabin.

The Captain's Cabin

Captain's Cabin aboard the Sea Serpent's Bane

Read aloud

The cabin is a wreck—charts torn, furniture overturned. In the corner, a heavy lead-lined chest sits bolted to the floor. Captain Voss stands over it, her hand trembling. 'It's in there,' she whispers. 'It sings to me. I can't... I can't think when it's open.'

Running this scene

The idol is a small, four-faced statuette made of iridescent stone. It radiates an aura of enchantment. Anyone who opens the chest must make a DC 15 Wisdom saving throw or be affected by the confusion spell (save ends). The captain explains that they found the idol on the Isle of Whispers, in the wreck of another ship. A cloaked figure was seen leaving the island. The idol has a symbol of a four-headed dragon. Captain Voss is desperate to be rid of it. She asks the party to take it to Arcanist Melvin, a local wizard, for safekeeping. If the party examines the idol, they can learn more.

Checks
  • Intelligence (Arcana) DC 15 to identify the idol as a 'Mind-Rending Idol', a cursed object that amplifies elemental energy and breaks minds.
  • Wisdom (Insight) DC 14 to notice Captain Voss is hiding her fear of the idol's origin.
  • Intelligence (Investigation) DC 13 to find a logbook entry mentioning the Isle of Whispers and a 'cloaked stranger'.
Clues
  • The idol is one of many, created by a lich to summon a four-headed elemental dragon.
  • The Isle of Whispers is a known hazard, but no one has returned from it in years.
  • The cloaked figure matches descriptions of cultists of the Four Winds.
Where next
  • Captain Voss asks the party to deliver the idol to Arcanist Melvin's tower in the city.
  • The party can also choose to keep the idol, but it will attract cultists.

The Arcanist's Tower

Arcanist Melvin's Tower, Tidewatch

Read aloud

The tower is a jumble of leaning bookshelves and bubbling flasks. A gnome with wild white hair looks up from a tome. 'Visitors! And you've brought... oh my. Is that a Mind-Rending Idol? How thrilling! And terrifying! Come in, come in!'

Running this scene

Arcanist Melvin is a whimsical, absent-minded wizard (use the Mage stat block if needed, but he is non-hostile). He identifies the idol and explains that it is a fragment of a larger set, created by a lich named Malakar the Soulbinder. Malakar seeks to summon the Cataclysm, a four-headed dragon representing the four elements, to control the world. The idols are being seeded across the land to gather elemental energy.

Melvin can detect a faint magical trail from the idol leading to the city sewers. He offers to keep the idol safe in a lead-lined vault, but warns that the cult will come for it. He also mentions that the cult is known as the Cult of the Four Winds.

Checks
  • Intelligence (Arcana) DC 16 to learn more about Malakar and the Cataclysm.
  • Charisma (Persuasion) DC 13 to convince Melvin to help the party without payment.
  • Wisdom (Perception) DC 14 to notice a shadowy figure watching the tower from an alley.
Clues
  • Malakar the Soulbinder is a lich who seeks to control the Cataclysm.
  • The Cult of the Four Winds is spreading the idols.
  • The magical trail leads to a hideout in the sewers beneath the docks.
Where next
  • Melvin gives the party a potion of healing and a map to the sewer entrance.
  • The party can follow the magical trail or the shadowy figure (a cultist spy) to the hideout.

The Cult of the Four Winds

Sewer Hideout beneath Tidewatch

Read aloud

The sewers reek of rot and incense. Ahead, a faint orange glow flickers, and a rhythmic chanting echoes off the stone. A symbol of a four-headed dragon is painted in blood on the wall. You hear a voice: 'The Ember shall rise! The Cataclysm comes!'

Running this scene

The hideout is a small shrine to the Cult of the Four Winds. The leader is Kaelen the Ember, a Cult Fanatic who wields a fire-themed idol. He is guarded by 4 Cultist. Kaelen is a pawn of Malakar, tasked with spreading the idols and testing their effects. He will fight to the death, but if captured, he can be interrogated. He knows little of Malakar's grand plan, but he has a letter from the lich with instructions to send the next idol to a contact in the city of Emberfall. The letter also mentions 'the Nexus' and 'the four heads'. After defeating Kaelen, the party can recover the letter and a second idol (fire-themed).

Checks
  • Dexterity (Stealth) DC 14 to sneak into the hideout unnoticed.
  • Strength (Athletics) DC 12 to force open a barred door.
  • Intelligence (Investigation) DC 15 to find the hidden letter in Kaelen's quarters.
  • Charisma (Intimidation) DC 14 to interrogate a captured cultist.
Clues
  • Kaelen carries a letter from Malakar mentioning a contact in Emberfall.
  • The letter references 'the Nexus' as the site of the summoning.
  • A second idol (fire-themed) is found, confirming there are multiple artifacts.
Where next
  • The party can return to Melvin or Captain Voss with the letter and idol.
  • The letter provides a lead to Emberfall, setting up Act II.
  • The act ends with the party reaching level 5 and realizing the scope of the threat.

Milestone: The party reaches level 5 after defeating Kaelen and recovering the letter, gaining a clear lead to the next stage of the lich's plan.

Villain Plans

Kaelen the Ember Character Sheet

Cult Leader of the Four Winds in Tidewatch

Motivation

Kaelen is a fanatic who believes the Cataclysm will cleanse the world of weakness and that Malakar will reward him with power. He seeks to prove his worth by spreading the idols and causing chaos, hoping to be elevated to a fiery general in the new world order.

Personality

Arrogant, zealous, and quick to anger, but secretly insecure. He speaks in fiery metaphors and enjoys dramatic speeches. He is easily flattered and will monologue if given the chance. He fears being forgotten or deemed unworthy by Malakar.

Resources

  • 4 Cultist followers
  • Sewer hideout shrine
  • Fire-themed idol (Ember's Brand)
  • Letter from Malakar with instructions
  • Knowledge of the Cult's next target: Emberfall

Battle Plan

Kaelen opens combat with a dramatic speech, then uses his fire spells (burning hands, scorching ray) to target spellcasters or clustered foes. He commands his cultists to protect him and flank enemies. If reduced to half HP, he becomes desperate and may use a potion of healing or attempt to flee, but his fanaticism usually keeps him fighting. He will not surrender unless captured and shown proof of Malakar's betrayal.

Defeat or Redemption

Kaelen can be killed in combat, or captured and interrogated. If captured, he can be persuaded (DC 18 Charisma (Persuasion)) to reveal what he knows about Malakar, though he remains loyal. A truly convincing argument (DC 20) might turn him against Malakar, but he is a fanatic and unlikely to change. If killed, his last words curse the party and praise the Cataclysm.

Running Notes

Recommended party: Four characters, any composition, but a balanced mix of combat, social, and exploration skills is recommended.

Session Zero

Before play, discuss the campaign's mix of dark horror and whimsical humor. Establish safety tools (e.g., X-card, lines and veils) because the idol's mind-rending effects and cult violence can be intense. Clarify that the campaign is not nautical-focused; the ship is just the opening. Encourage players to create characters with ties to Tidewatch or reasons to be at the docks. Agree on a light-hearted tone where silly side quests and eccentric NPCs are welcome. Ensure everyone is comfortable with themes of madness, cults, and undead.

Scaling Guidance

Act I is designed for a party of four level 3 characters, ending at level 5. For a larger party (5-6), add 1-2 Commoners to the docks encounter and 1-2 Cultists to the sewer hideout. For a smaller party (3), reduce Commoners to 2 and Cultists to 3. If the party is level 4 at the start, increase the Thug to a Bandit Captain (or add a second Thug) and give Kaelen an extra spell slot. If the party struggles, have Captain Voss or Arcanist Melvin provide a potion of healing or a timely distraction. The idol's confusion effect can be reduced to a DC 13 save if the party is overwhelmed.

DM Advice

Act I should take 3-4 sessions. Start with the chaotic docks scene to immediately engage the players. Roleplay Captain Voss as desperate but proud, and Arcanist Melvin as delightfully absent-minded. Use the idol's whispers to build tension. The sewer hideout is the climax; make Kaelen a memorable fanatic. If players get sidetracked, use whimsical encounters (e.g., a talking seagull, a pie-eating contest) to keep the tone light. The letter from Malakar is the key to Act II; ensure they find it.

Secrets & Clues

  • The Mind-Rending Idol is one of many created by Malakar the Soulbinder, a lich who seeks to summon the Cataclysm, a four-headed elemental dragon.
  • The Cult of the Four Winds is seeding these idols across the land to gather elemental energy for the summoning.
  • Kaelen the Ember carries a letter from Malakar that mentions a contact in the city of Emberfall and references 'the Nexus' as the site of the summoning.
  • The idol was found on the Isle of Whispers, where a cloaked figure was seen leaving the wreck of another ship.

Encounters

Mutiny on the Sea Serpent's Bane

Suggested Level: 3

The party must subdue or calm the maddened crew of the Sea Serpent's Bane as it docks. Three sailors fight each other or leap overboard, while the first mate attacks anyone who approaches.

Creatures

Terrain Features

The ship's deck is slick with rain and blood. Ropes and rigging provide cover and can be used to swing or trip. The edge of the ship is a hazard: falling overboard requires a DC 10 Strength (Athletics) check to avoid being pulled under by panicked sailors.

Treasure

None, but Captain Voss offers 50 gp for help once the crew is subdued.

DM Notes

The sailors are not evil, just magically confused. Non-lethal damage or skill checks can end the fight without killing. The first mate (Thug) is more violent but can be knocked out. If the party kills crew members, Captain Voss is distressed and the reward is halved.

The Cult of the Four Winds

Suggested Level: 4-5

In a sewer shrine, Kaelen the Ember and his cultists perform a ritual to empower a fire-themed idol. They attack intruders on sight, chanting praises to the Cataclysm.

Creatures

Terrain Features

The sewer hideout is cramped, with narrow walkways over foul water. Braziers provide dim light and can be knocked over to create smoke or fire hazards. A blood-painted symbol of a four-headed dragon covers one wall.

Treasure

A fire-themed idol (magic item), a letter from Malakar, and 150 gp in mixed coins.

DM Notes

Kaelen fights fanatically, using fire spells and commanding cultists. He can be captured and interrogated. The letter is hidden in his quarters (Investigation DC 15). If the party struggles, reduce cultists to 2 or have Kaelen flee to fight another day.
