Faces of the Maw

Rare Loot
Value 3255 gp
Type Wondrousitem
Attunement Required
This grotesque mask is made from repurposed flesh and bone, resembling the distorted visages of countless creatures. While worn, the mask allows the user to tap into the chaotic essence of the Maw of the Many-Faced Aberration. Once per long rest, as a bonus action, the wearer can choose to gain advantage on one attack roll, skill check, or saving throw. Additionally, if the wearer successfully deals damage to a creature, they regain hit points equal to half the damage dealt. However, the wearer must succeed on a DC 13 Wisdom saving throw after using this ability or become frightened until the end of their next turn as whispers of the fused souls fill their mind. Furthermore, while wearing the mask, once per long rest, you can cast the spell 'Confusion' (save DC 15) without expending a spell slot, reflecting the disorienting nature of the aberration's many faces.

In addition, the wearer has resistance to psychic damage, embodying the Maw's strange resilience. The mask has five distinct faces that shift and change, each representing a different expression, and the wearer can roll a d4 at the start of their turn to determine which face grants an additional boon: 1) Advantage on Charisma checks; 2) Advantage on Intelligence checks; 3) Advantage on Dexterity saving throws; 4) Advantage on Constitution saving throws. The mask requires attunement by a creature with an evil alignment, who is susceptible to its whispering madness.
