Overview

A banished archmage, scorned by a world that outcast his dangerous experiments, seeks vengeance by unleashing a plague-god upon the realm. He uncovers a secret tome, the Grimoire of the Rotveil, which unlocks a portal to Nerathis, the Bloom of Pestilence. Nerathis grants him power to sow fear and disease and to recruit followers who crave a world reshaped by rot.

Setting and Tone

The campaign unfolds across fog-slick coastlines, mist-washed swamps, and ruined keeps. Factions include a wary Church of Purity, a mercantile guild wary of plague, and scattered epistolary libraries that hoard forbidden lore. The threat twists through cities, farms, and dungeons as the illness-bearing mists creep forward.

Main Quest

Track Aldren Kaelor from the Crumbling Spire to the Rotspire Citadel. Intercept three ritual nexuses that open Nerathis’s influence: a plague-wreathed tomb, a corrupted aqueduct shrine, and a diseased crucible in the Blighted Forge. Gather diverse allies, sever the cult’s chain of recruitment, and perform a counter-ritual to seal the portal before Nerathis can fully manifest in the material realm.

Adventure Arcs and Hooks

  • Act I: Emergence. Outbreaks disrupt trade and travel. A dying plague-doctor seeks aid and hints at Nerathis’s symbol appearing across town gates.

  • Act II: Recruitment and Rivalry. The cult targets merchants, nobles, and mages; players must foil assassinations, win over wary folk, and uncover the tome’s location before Aldren does.

  • Act III: Ritual and Reckoning. The party discovers Nerathis’s true demands: fear as currency. Their choice at the climactic nexus shapes the realm’s future—seal the portal, or barter a fragile peace with the plague deity.



Challenges

Creeping disease hazards, plague-sworn cultists, undead minions, corrupted wetlands, and moral quandaries about whether some cures are worse than the disease. The journey tests diplomacy, stealth, and bold counter-ritual magic rather than brute force alone.

Endgame and Rewards

Victory yields powerful artifacts: the Starless Mantle (resists disease and masks the bearer’s auras), the Wyrmroot Dagger (banishes diseased foes), and the Purifier Sigil (grants temporary immunities to spell-laden plagues). Even in triumph, players may bear the moral scars of choosing mercy over vengeance—and the world forever bears Nerathis’s shadow.
